Example run report
A run report shows what the calculator counted, what it did not count, and why. This example is the breast cancer screening measure (CMS125, 2026) run on a live eClinicalWorks FHIR R4 bulk export of 1,339 patients and about 1.8 million resources, evaluated on fqm-execution. It holds counts only: no patient identifiers.

Section titled “How to read it”Counting modes
Section titled “Counting modes”- Reporting mode counts what the source coded, plus repairs, plus signed rules.
- Quality-improvement mode also counts unsigned derivations and inferences, shown as a hatched segment so you can see which part of the rate rests on them.
Three runs of the same measure
Section titled “Three runs of the same measure”| Run | Initial population | Numerator |
|---|---|---|
| As published, no rules | 278 | 0 |
| Conformed, reporting mode | 286 | 0 |
| Conformed, quality-improvement mode | 425 | 291 |
| Production calculator, for reference | 427 | 291 |
The published package scores zero. Mammograms in this export are coded only with the site’s radiology order text, and the inference that maps that text to LOINC is not yet signed.
The account
Section titled “The account”Each row is one case the rules affected, with:
- the population it affects;
- the empty retrieve and the missing element;
- the owner of the fix (Standards, Vendor or Site);
- the rule that supplies the element, its class, and whether it is signed.
Filter by owner, rule class, signed status or population.
The rules in this run
Section titled “The rules in this run”| Rule | Class | Tier | What it does |
|---|---|---|---|
| HCPCS code system relabel | Repair | Vendor | HCPCS codes labelled as CPT are relabelled to HCPCS. The code is unchanged. 22,525 codings, 8 patients. |
| Clinical sex from administrative gender | Inference | Vendor | Adds the US Core sex extension, inferred from Patient.gender. 139 patients. |
| Onset from recorded date | Derivation | Vendor | Fills a missing Condition.onsetDateTime from recordedDate. 63 conditions, no population changed. |
| Radiology order text to LOINC | Inference | Site | Maps seven radiology order names to LOINC mammography codes across 3,851 reports. 291 patients. |
One proposed mapping was rejected. “Transfer of outside films” is an administrative act, not a mammogram, and would have added 120 false numerator events.
Reconciliation with the production calculator
Section titled “Reconciliation with the production calculator”The production calculator has 427 eligible patients and the conformed run 425. Five patients are counted only by the production calculator and three only by the conformed run, all on plain office visits in the Office Visit value set. The numerators match at 291.
